We will be happy to help you. The insurance companies require the applicant to undergo a medical examination when signing or moving, e.g. life insurance, endowment insurance, and pension insurance. Before the medical examination, the insurance company must send out a document that often contains two parts, a part that the customer fills in himself and a part that the examining doctor fills in. 

In addition to a medical examination, blood and urine samples and an EKG are often included.
